Meets the education requirements for certification and is in the process of accumulating the supervised experience required for certification. • Provide individualized evaluation, comprehensive planning, development of goals, implementation of treatment and discharge plans, documentation of findings and progress, and patient/family education in the home setting under the direction of a licensed physician. • Bachelor of Science degree or Master of Science degree in Physical Therapy from an APTA accredited program. • Current licensure in MA and/or NH. • Two years of experience preferable; in acute care, rehabilitation or home health including age related competence for pediatric adolescent, adult, and geriatric patients as indicated. • Knowledge of growth and development across the life span. • Certification in BCLS of Adult. • Must have valid driver’s license and access to reliable transportation. • National and State Physical Therapy Association membership preferred. • Provide individualized evaluation, documentation of findings and progress, comprehensive planning, and implementation of treatment plan, patient/family education and determination of rehabilitation and patient goals under the direction of a physician. • Bachelor's or Master's Degree in Occupational Therapy from an AOTA-accredited program. • Current licensure by the Board of Allied Health Professionals, Commonwealth of Massachusetts. • American Occupational Therapy Association Membership preferred. • Current Certification in BLS of Adult, Child and Infant. • Knowledge, experience, and proficiency in dealing with Neuro, adult, and pediatric preferred. • Serves as coordinator of transportation to and from therapy. • Ability to follow directions, coordinate a variety of responsibilities and receive supervision from several sources. • Good organizational and communication skills. • Interpersonal ease with empathy for adult rehabilitation and geriatric patients. • Current Certification in BLS of Adult, Child and Infant.
